Before you fill out the Volunteer Application, please read below:

The Volunteer Application has been formatted to facilitate completion by hand and electronically using Adobe Acrobat Reader only. Adobe Acrobat Reader is free to download.

*** ATTENTION MAC USERS ***
Please ensure that you are using Adobe Acrobat Reader. Other PDF viewers such as MAC Preview and PDF Exchange will render the form unreadable when submitted to Polk County.

Volunteer applications received that are unreadable, incomplete, or received blank due to software or system incompatibility will not be processed.

  1. Be sure Adobe Acrobat Reader is downloaded on your computer.
  2. Select the appropriate Volunteer Application link listed below to download the application.
  3. Select “save” (Save the volunteer application to your computer rather than attempting to edit it within the browser window which may cause complications due to internet browser’s default settings.)
